It will include discussion on why process hazard analyses are ignored or not followed-up; how in some cases front line employees had warned about risks that preceded incidents (or prevented them); current management health and safety programs that include employee participation as a key element; and recommendations from Chemical Safety Board investigations that can enhance worker engagement.	en
